Affiliated Instructors: K-12 Special Record Submission
Immediately following your class, complete a fillable Excel K-12 Course Roster file (click to download a blank copy). Rosters in this fillable Excel document is the only format that will be accepted.
Every student on the roster must have a First Name, Last Name, separate/unique Email Address, and a Phone Number. A unique email address belonging to the student is required for each person - a single school email can’t be used for multiple students. If a student did not pass the class or did not finish the course, do not include their information on the roster you upload.
Once you have filled in the Excel Roster, submit it using the form below. After submitting the roster below, you will be taken to the payment page to pay for your students. Payment must be made in full after roster submission in order for Ohio CPR Certs to process you student certifications.
Additional requirements must be met in order to receive the special K-12 pricing:
Courses must be taught on-site, at a K-12 school location. Courses taught off-site from a physical school address will not receive K-12 pricing. Class addresses must be submitted on the course roster for verification.
EX: Teaching a class at a local youth center would not be eligible simply because the students happen to be K-12 aged.
All participants taking the course must be students, faculty, or staff members of the K-12 school where the course is being taught. Members of the general public may not take a special K-12 course, even when in combination with students/faculty/staff.
EX: Teaching a scout troop that meets at a school would not be eligible. Teaching a babysitting course open to the public in the school cafeteria would not be eligible. Teaching a course with school students and their parents would not be eligible.
K-12 pricing is limited to the course titles listed in the chart below, variations on these courses are not permitted.
EX: Lifeguarding and Emergency Oxygen would not be eligible
Courses may be taught in the blended learning or fully in-person format.
Certification cards will have the “Ohio CPR Certs K-12” name listed as the provider – affiliated organization names are not able to be listed on special K-12 certifications.
K-12 courses are subject to additional scrutiny and evaluation by the American Red Cross QA department. The Red Cross will contact students from time to time to evaluate for course length, course content, location, and affiliation to the K-12 school. Instructors found to be violating the policies surrounding K-12 course pricing are subject to instructor certification revocation, and any certifications issued by the instructor to be voided.
