Affiliated Instructors: Record Submission

Immediately following your class, complete a fillable Excel Course Roster file (click to download a blank copy). Rosters in this fillable Excel document is the only format that will be accepted.

Every student on the roster must have a First Name, Last Name, separate/unique Email Address, and a Phone Number. A unique email address belonging to the student is required for each person - a single company email can’t be used for multiple students. If a student did not pass the class or did not finish the course, do not include their information on the roster you upload.

Once you have filled in the Excel Roster, submit it using the form below. After submitting the roster below, you will be taken to the payment page to pay for your students. Payment must be made in full after roster submission in order for Ohio CPR Certs to process you student certifications.
